Data Collection Form Options

The Form SF-SAC for fiscal periods ending in 2004, 2005 and 2006 was extended to include 2007. The data entered on-line for 2007 audits will be automatically entered on the 2004 Form on-line.

2008 Form SF-SAC Notice - The Form SF-SAC for audits with fiscal periods ending in 2008 will be available on-line in the Summer of 2008. If a 2008 audit is completed before the 2008 Form SF-SAC is available, auditees may not be able to meet the thirty day deadline for submission. If that situation applies, auditees must request extensions from the Federal oversight or cognizant agency. Federal agency contact information is available at http://harvester.census.gov/fac/APPX3.htm .

The FAC is not authorized to grant extensions of the deadline for filing OMB Circular A-133 submissions. The auditee should contact their cognizant/oversight agency to request an extension. For information on cognizant or oversight agencies, please refer to Sections 400a & 400b of Circular A-133.
